Dera Ismail Khan is the last big town of Khyber Pakhtunkhwa before the river turns the map toward Punjab. People shorten it to D.I. Khan. The “Dera” in the name is the old word for a camp or settlement. Ismail Khan was the sixteenth-century founder the town still carries. From here the road goes north toward Tank and the Waziristan districts, east across the Indus toward Bhakkar and Mianwali, and west into the dry tehsils of Kulachi and Daraban.

This is not Swat and it is not Abbottabad. Summer is long. Dust and canal water decide the crop more than snow. Saraiki is strong in the city and the eastern villages. Pashto is strong toward the western tehsils and among families who moved in from the hills. Both languages sit in the same bazaar. A student page that pretends the district is only one of those languages is already wrong.

Dera Ismail Khan: What the 2023 census actually printed

Citypopulation compilations of the PBS 2023 tables give the district 1,829,811 people on about 9,334 km². That is roughly 196 people per square kilometre — dense in the city tehsil, thin in Drazanda. Males 959,405, females 870,255, transgender 151 in that compilation. Children under ten are a large share (the 0–9 row is over half a million). Household size on the official Table 1 style figures for this belt is high, in the six-to-seven range.

Urban names in the same 2023 municipal rows: Dera Ismail Khan municipal council about 213,756; cantonment about 6,819; Paharpur municipal council 76,027; Paroa 42,005; Kulachi 24,481; Paniala town council 11,669. Add those and you still have a district that is mostly villages and canal colonies, not a second Peshawar.

Literacy for the old wide district is often quoted near 46–47 percent overall. Tehsil by tehsil the gap is sharp: Dera Ismail Khan tehsil about 57 percent, Paharpur about 49 percent, Paroa about 36 percent, Kulachi about 30 percent, Drazanda about 29 percent, Daraban about 27 percent. Female literacy sits well below male literacy in every one of those rows. That is the figure worth remembering, not a slogan about “education city”.

One administrative warning belongs at the top. Some 2024–25 lists treat Paharpur as a separate district. The 2023 census book still prints Paharpur as a tehsil of Dera Ismail Khan. Until PBS issues two finished district volumes, this atlas keeps Paharpur on this page and marks the split as a live notification, not as a second completed census.

Dera Ismail Khan: Tehsils on the 2023 sheet

Dera Ismail Khan tehsil — 1,167 km², 767,979 people, about 658 per km². This is the city, the cantonment, Gomal University, Gomal Medical College, and the teaching hospitals. Twenty-one union councils on older lists. If a reader only ever sees this tehsil, they will think the whole district is a university town. It is not.

Paharpur tehsil — 1,657 km², 406,467 people, literacy about 48.9 percent. Eleven union councils on those lists. Second-largest population after the city tehsil. Paharpur town is the municipal name. THQ Paharpur is the hospital people in that belt use before they take the road into Dera. If the new district notification sticks, this paragraph should later move to its own page.

Paroa tehsil (also written Parowa) — 1,733 km², 320,937 people, literacy about 35.6 percent. Seven union councils on older lists. Municipal council about 42,000. Type-D / THQ-style facility at Paroa is in the public health lists. The land here is canal and katcha together; flood years rewrite the same fields.

Daraban tehsil (Darabin in some spellings) — 1,540 km², 149,447 people, literacy about 26.9 percent. Four union councils on older lists. Civil Hospital Daraban Kalan is the named public facility. This is dry country compared with the city belt. Distance to the teaching hospital is the daily fact.

Kulachi tehsil — 1,229 km², 102,595 people, literacy about 30.3 percent. Four union councils. Kulachi municipal council about 24,500. THQ Kulachi is the local hospital name. Population barely moved between 2017 and 2023 in the compiled table (101,892 to 102,595). That flat line is unusual next to Paroa’s earlier jump and is worth keeping as a census note, not a mystery essay.

Drazanda (Drazinda / Darazinda; former Frontier Region D.I. Khan) — 2,008 km², 82,386 people, about 41 per km², literacy about 28.7 percent. Civil Hospital Darazinda is on the public facility list. This is the western, thinly settled side that used to sit outside the old settled district. Treating it as an afterthought is how a Dera page goes wrong.

Paniala appears as a town council and, on some tehsil lists, as its own tehsil line still being filled. Until PBS prints a clean Paniala tehsil row, name the town and do not invent a full tehsil table.

Dera Ismail Khan: City life and the river

The municipal town sits on the west bank of the Indus. Circular Road, the old gates, and the cantonment are the landmarks a visitor actually uses. The river is both a market and a threat. When the Indus and the hill torrents rise, Paroa and the katcha villages take the first water. After the flood the same families rebuild on the same ground because that is the farm they have.

Gomal University is the degree name that belongs to this city. Gomal Medical College (public, KMU-affiliated, working since the late 1990s) is why a student from Tank or Kulachi can sit an MBBS seat without disappearing into Peshawar on day one. Affiliated teaching names in the public record include DHQ Teaching Hospital and Mufti Mehmood Memorial Teaching Hospital. The Women and Children / Zanana hospital is a separate building people still call by the old name.

In late 2025 the DHQ side publicly described a jump from a 242-bed hospital toward a 600-bed notification, a cath lab project, a kidney-centre scheme, and a mammography unit. Those are hospital-director statements in the press, not a PBS table. They are reported here as projects and claims. Check the next official MTI notice before treating bed counts as frozen.

THQ and civil-hospital names already on government facility lists: THQ Kulachi, THQ Paharpur, Type-D Paroa, Civil Hospital Daraban Kalan, Civil Hospital Darazinda, Type-D Paniala, RHC Kot Jai, RHC Kiri Shamozai. That list is enough to show the district is not one building on Circular Road.

Work in Dera Ismail Khan District

Wheat, sugarcane and livestock pay the villages. The city pays wages in shops, transport, the university, the hospitals and the offices of the division. Traffic toward Zhob and toward Punjab is a living. When the river road is open, Dera feels like a junction. When it is not, the western tehsils wait.

This is not a tourism paragraph. There is heritage — old gates, the river-front, the university lawns — but the daily story is heat, school attendance after Class 8 for girls in Daraban, and whether the THQ has a lady doctor on Tuesday.

Dera Ismail Khan: What this page will not pretend

It will not add Paharpur’s 406,000 people twice if a new district gazette is already on a government website. It will not list every union council as if 47 names were 47 complete village books. It will not use the division total (over three million, including Tank and the Waziristan districts) as if that were the district. Division and district are different maps.

Send a village name from Paroa, Daraban or Drazanda if you live there. That is how this atlas is supposed to grow.
